Documents Required for LIC Proposal Form:

Any Proposal Form of LIC has to be filled when you buy a New Life Insurance Policy from Life Insurance Corporation of India (LIC). Therefore, LIC's Proposal Form is a very important document for LIC and for the customer.

The customer can submit his Aadhar Card, Driving License, Passport, Voter ID Card, etc. as proof of residence.
